This Google Web App requests access to your Google Account for the following reasons:
Create and store a Google Form in your Google Drive. Every Google Account has space for the storage of multiple types of data. Your GMail, Sheets, Documents, Photos... and Forms are all stored with your Account. In order for this tool to create your customized Form, it needs to be stored along with your other Account information.
Create and store a Google spreadsheet (Sheet). This Google Sheet is linked with your Google Form and is used to collect the data from your members as they submit their responses to the Form.
After this tool creates the Form and its linked Sheet, two URL's are displayed. In case you miss recording those two links, they can be sent to you via email. This tool requests access to your email, for a single purpose only... to send email on your behalf. This tool does not have access to *read* your emails, only to send as you.
This tool has been carefully and intentionally written with "minimal permissions" design principles, and with the security of your account and information in mind. You are the only account who can access your Google Drive, Forms, Sheets, and GMail (*send* only).
The developer of this tool does not have access to your Google Account or any of its information.
If you wish to revoke any permissions that you have previously granted to this tool, you may use Google's Account management tools.
To revoke permissions from this app., use this link: https://myaccount.google.com/permissions and select the app. you wish to adjust. You will see an option to "REMOVE" permissions.
