Customize the form's theme

1. Open your new form using the link provided. Since you are the creator of the form, you will have the "EDIT" capability. Regular users of your form do NOT have the ability to edit the form.

2. Click the "pencil" icon (shown by the green arrow below)

3. Choose the "Color Palatte" icon to select a theme. You can choose a standard color, or (in this example) we will choose the "custom" theme.

4. From the custom theme icon, you can select to upload your own photo

5. In our example, we upload the F.A.T. Masthead from: Free Toast Host

Congratulations! You now have a themed, customized Google Form -ready to collect your member's responses.
